Refer for a Joint Counseling Session

This form can be submitted by an agency or by the parties themselves. It is used to request a joint counseling session as part of a third-party reproduction arrangement—commonly involving intended or recipient parents and a known egg donor, sperm donor, embryo donor, or gestational carrier.
